TWCC-73 Instructions

The form  presented here was drawn from that produced by the Texas Workers Compensation Committee.  

To add or edit data

Simply click in one of the data fields (e.g. the "Injured Employee's Name" box).  

You can then navigate through the form fields either by clicking with your mouse or by pressing the Tab key. 

Be sure that you have the hand button selected in your Acrobat viewer to be able to edit text fields.

To Zoom In

Press the zoom toolbar button in your Adobe Reader, then click on the page where you want to zoom.  Be sure to reselect the to be able to edit text fields again.

Note you can also quickly zoom in or out by using the zoom control at the bottom of the Adobe window.

If you can't see the form

Wait a few minutes to ensure it's completely downloaded.  If you don't have Adobe Acrobat Reader version 4 installed on your system, you can download it free from Adobe.
